For the last four years of their six-year relationship, Prokop had choked her and even threatened her life, among other things.

According to TMZ, Hyland's fellow "Modern Family" actress and onscreen mother Julie Bowen witnessed some of this abuse and helped Hyland when she ended the relationship in August.

Prokop was very distraught by the breakup and "relentlessly bombarded [Hyland] with vile, threatening and emotionally disturbing texts and voice mails including his own suicide threats," the "Modern Family" actress said in the documents found by TMZ.

The actor went to rehab in August, but got out on September 21. After being contacted by the facility director, Hyland then filed a restraining order against Prokop, requiring him to stay 100 yards from her and her dog. The restraining order was granted on September 19.
